In Network

As a courtesy, ICS will submit claims to insurance carriers for which we are an in-network service provider.  However, when using health insurance benefits, there are still likely to be some out-of-pocket costs such as unmet deductibles, coinsurance, copayments, and professional services not covered (e.g. services related to legal matters, reports, etc.).   

Out of Network/PPO

For all other insurers with an out of network benefit, or if you have a PPO plan that provides flexibility to work with non-network providers, fees will be due in full at the time of service.  However, ICS will happily provide monthly receipts documenting expenses incurred.  The receipt will include all information requested by your insurance company for the purpose of assisting you in obtaining reimbursement.
